Forms
Let members fill out a gated form.
A form reward lets members who hold a role fill out one of your gated forms: applications, surveys, sign-ups for an event, feedback from holders only. You build the forms in the admin's Forms module and read the responses there.
Forms are part of the Pro plan and above. See Plans.
Create the form
- In the admin panel, open Forms and create a form with the questions you need.
- Optionally set Submissions per member. 0 means unlimited, and rejected responses don't count toward the limit.
Gate it with a role
- Open Roles, select the role that should be able to fill out the form, and under Unlocks press Add reward.
- Pick Form (v2).
- Under Choose a form, pick the form. Only forms that aren't linked to a reward yet are listed. If there are none, Create new form takes you to the Forms module.
- Decide on Automatic approval. On, every submission is approved immediately. Off, submissions wait for you to approve them.
- Press Add form reward.
How it works for members
Members who hold the role see Open form on the reward card and fill it out. Members without the role see it locked. If they lose the role, they can no longer fill out the form.
Review responses
In Forms, open the form to see its Responses. Approve or Reject each one when approval isn't automatic, and download them from Export responses.
Turn a response into a role
Pair a form with the Submitted a form requirement to make applications unlock a role. Only approved submissions count, so with Automatic approval off you decide who gets in. For example: a public "Applicants" role unlocks the "Apply to Core" form, and the "Core" role requires Submitted a form for it. See Guild activity requirements.
